Gout is the most common metabolic disease that expresses itself in painful joints or the linings of tendons. It is responsible for the red big toe called a “podagra”. This refers to the exquisite pain that people experience with acute gout attacks, usually in the first few days. The reality, though, is that gout can flare up in any joint and is very commonly seen in and around the hand, specifically in the wrist joint.
Gout is a metabolic condition and is primarily caused by an accumulation of uric acid in the body. Uric acid is a type of waste formed when the body breaks down purines, which are substances found in certain foods and naturally produced by the body. Normally, uric acid dissolves in the blood and is excreted through urine. However, in individuals with gout, there's an excess of uric acid that forms sharp, needle-like crystals in the joints and other tissues. The body recognizes these crystals as foreign and subsequently launches a very aggressive, acute inflammatory attack by cells that are driven by the neutrophils. It is these same cells that we usually see in acute bacterial infections, and therefore, the entire inflammatory process is synonymous with pain, swelling, redness and essentially a disuse of the joint. In the upper limb, the gout expresses itself through an acute painful wrist. Ultimately, patients with chronic gout develop what is called “gouty tophus”, which is a white toothpaste-like substance caused by the buildup of uric acid. These crystals, or top houses, are locally destructive and can destroy the joint cartilage, the ligaments, soft tissue and bone.
Treatment
The treatment of gout starts with addressing the metabolic conditions. Physicians and General Practitioners first look at any metabolic components associated with lifestyle and diet. The diagnosis of gout is usually confirmed with blood tests that test the uric acid levels. If these are raised, patients are typically treated with a medication that will lower the uric acid levels. In acute situations, we try to treat this conservatively as far as possible, and in chronic cases where there is a buildup of tophus and local joint destruction, reconstructive surgeries are considered.
In summary, gout is a metabolic condition with the buildup of uric acid crystals, which can cause destruction of local tissues.
